Copy of the applicant's passport.
FS1, FS2 and Year 1 applications - Copy of completed Doha College EYFS Form. No separate nursery report is required.
Year 2 to Year 10 applications - Most recent school report. (If the report is not in English, please provide a translation with the original.)
Immunisation records.
A copy of the applicant's and the sponsor's Qatar Residence Permit (QID.) If this is not available at the time of application, please complete the form to state ‘In Process’
A passport size photograph (optional)
Any relevant medical and educational psychologist reports, if applicable.
